Patient continues to progress nicely in rehab and is tolerating her advanced diet to Mechanical Soft and Thin liquids without any unplanned hospital transfers.

Under the Direction of our In-House Cardiologist, Dr. Dhirendra Das, the patient gained the strength required to return home. Follow up appointment secured with PCP, Dr. Connolly within 48 hours of Discharge from Rehab. He will continue to follow with Community Cardiologist, Dr. Anthony Losardo, is set up with Meals on Wheels and VNA of Central Jersey.

Upon admission patient was dependent, requiring maximum assistance with all self-care tasks. The team approach to care including the support of our Full-Time on site Respiratory Therapist, allows the patient to participate in Occupational, Physical and Speech therapy 5 times a week to achieve her goals.

Upon admission, the patient was bedbound requiring assistance with bed mobility, transfers, personal hygiene, toileting, and ambulation. The team approach to care including Full time Respiratory Therapy on site, allowed patient to participate in in Occupational, Physical and Speech therapy 5 times a week to achieve her goals. Upon discharge, patient was able to ambulate with supervision, eat independently, and ambulate with very minimal assistance.
