66 Year Old Female (L.M.) admitted to Allendale Healthcare and Rehabilitation Center after a stay at Valley Hospital and Kindred LTAC with Admitting Diagnosis of Acute Respiratory Failure with Hypoxia S/P removal of pancreatic mass and stent placement. Patient also presents with CVA& history of Diabetes, COPD and OSA.

Nursing Intervention:
Monitor Labs- BBG’s, CBC and BMP
Medication Management- Atorvastatin, Insulin, Midodrine and Lantus
Advance PO diet as tolerated and wean Peg tube as able
DVT prophylaxis- Lovenox

Respiratory Interventions
Maintain Patent Airway- wean as tolerated- Goal Accomplished
Maintain Adequate Oxygenation- Wean as tolerated-Goal Accomplished
Deep Breathing and Cough- Maintain Pulmonary Hygiene
Pacing and Endurance Training

Patient is seen weekly by our in-house Pulmonologist, Dr. Jason Shatkin. With the support of our Full-Time Respiratory Therapist our patient progressed quickly: Day 6-Trach Weaned
Patient continues to progress nicely in rehab and is tolerating her advanced diet to Mechanical Soft and Thin liquids without any unplanned hospital transfers.
