In January 2025, Atlantic Health, Clinical Care Consulting, Florham Park, Spring Grove and Oakland collaborated to develop a Pulmonary and Integrated Care Model. The teams established protocols and clinical pathways to strengthen subacute rehab management for patients with COPD and pneumonia. More than a year later, they are delivering strong outcomes, sharing expertise with other health systems and supporting patients with complex respiratory needs.
