Providence Rehabilitation & Healthcare Center in Yeadon, PA has introduced a new, comprehensive Chronic Illness Support care model, partnering with Keystone Care to integrate palliative care and pastoral support with its specialized clinical programming.
Providence Rehabilitation and Healthcare Center developed its patient-centric Chronic Illness Support based on a recently introduced platform. “Providence’s specialized care initiative emphasizes a holistic focus on both physical and spiritual wellness for chronically ill patients – it is truly a next-level model of care supported by an excellent partnership with Keystone,” said Danielle Koons, senior regional director of market development.
“Providence for years has provided exceptional care for those with chronic disease through its physician-lead specialty offerings,” noted Koons. “The facility’s new Chronic Illness Support model introduces a palliative care-focused overlay to further improve mental health and wellbeing for these patients and their loved ones. Ultimately, the program will support improved health outcomes and reduced hospitalizations within this patient population.”
The Providence care model – which specifically supports the facility’s physician-led Pulmonary Rehabilitation and Chronic Kidney Disease (CKD) Management programs – incorporates palliative care administered by a trained team of doctors, nurses and specialists. It focuses on providing emotional and spiritual support to help relieve symptoms and stress, and also weaves in disease-specific education and health coaching, social services support, life enrichment activities and connections to community resources.
Providence maintains a four-star rating from the Centers for Medicare & Medicaid Services (CMS). Located at 600 South Wycombe Ave., the facility is committed to serving as a healthcare resource for families of patients and the local community.
