ON-SITE HEMO-DIALYSIS ACCOMMODATIONS
72-year-old male patient (G.B.) admitted to Kearsley Healthcare and Rehabilitation Center after a hospitalization at Penn Presby Hospital with admitting diagnosis of Sepsis secondary to Covid Pneumonia, UTI. Patient is CKD to ESRD with an EF of 35%. He admits to Kearsley New to Dialysis.

Nursing Interventions:
Dialyze and Maintain Fluid Balance- 5 day a week onsite dialysis treatment; 1500ml Fluid Restriction
Monitor Vitals, Labs & Diagnostics- Potassium, Sodium, BUN, Creatinine, HGB, Mag
Medication Management- Novolog, Atorvastatin, Aspirin, Hydralazine, Isosorbide, Coreg, Procrit

On-site Hemodialysis and Cardiology lead weekly rounds made Kearsley the right choice for care for this patient.
Under the Leadership of our In-House Cardiologist’s, Dr. Droogan and Dr. Domsky, the team managed this fragile patient’s stay. Patient has gained strength and endurance needed to return to home with his wife without any unplanned hospitalizations.
He continues to require Dialysis and is New to Community Services Set up. Patient returns home with New Start Dialysis was set up with Davita West Phili and Penn Home Health. Patient continues to follow with PCP, Dr. Caitlyn Callaghan.
