This is a 70-year-old female patient admitted to Canterbury Rehabilitation & Healthcare Center with admitting diagnosis of acute respiratory failure with hypoxia, s/p trach and PEG from Vibra Hospital. Patient has a recent history of AKI, HTN, A-fib.
Nursing Interventions
Maintain patient safety
Glucose monitoring
Wound Care
Advance PO diet as tolerated- Wean Peg tube as able Monitor Labs- Tacrolimus Levels, CBC, BMP
Respiratory Interventions
Maintain Adequate Oxygenation- Weaned from oxygen to RA
Maintain Good Aeration-Bronchodilation and DB&C Exercises Trach wean- Tolerated capping trials and trach weaned within 4 weeks of her stay.
Therapy
Upon admission patient was dependent, requiring maximum assistance with all self-care tasks. The team approach to care including the support of our Full-Time on site Respiratory Therapist, allows the patient to participate in Occupational, Physical and Speech therapy 5 times a week to achieve her goals.
Patient continues to work with therapy and make slow gains. Patient enjoys the staff positivity and interactions, which has helped make the stay at Canterbury #NextToHome.
