Innovative Solutions in Challenging Times One of Jennifer’s most impactful initiatives was the development of the Urgent SNF program in 2015. “The program transformed how we addressed gaps in care,” she explains. During COVID-19, this innovative approach proved invaluable, facilitating thousands of direct transitions from hospital emergency rooms while supporting staffing-challenged home care agencies.
